Advanced Nurse Practitioner
Heart of Bath are looking for an Advanced Nurse Practitioner to join our large and friendly team.
The post holder will be an experienced nurse who, acting within their professional boundaries, will provide assessment, differential diagnosis and treatments of acute episodes of minor health and minor injury conditions.
The Advanced Nurse Practitioner is expected to see patients autonomously providing face to face, home visits and telephone consultations for patients within the Practice and within the patients residence and demonstrate the use of critical thinking in the clinical decision-making process.
The Advanced Nurse Practitioner will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as well as promoting self-care in order that patients are empowered to manage their own healthcare needs. Support from the Duty doctor will be available at all times if needed.
