Ordering your prescriptions

Prescriptions can be ordered through any of the following routes:

  • Online via the NHS App
  • Online via the SystmOne repeat management website
    (see online services to register for this service)
  • Email to bswicb.heartofbath@nhs.net
  • Post to Oldfield Surgery, 45 Upper Oldfield Park, Bath BA2 3HT
  • Drop your repeat request slip into the surgery.

We do not accept medication requests over the phone.

Please avoid using the appointment triage forms to request your medication as it may take longer to process.

If you feel you are unable to order your own prescriptions or are the carer for somebody who is unable to order their own prescriptions, please ask your nominated pharmacy who can support you in ordering prescriptions.

Prescription Ordering Direct (POD) service

Unfortunately, the POD service is being withdrawn completely across BANeS, Swindon and Wiltshire.

From Sunday 21st April, Heart of Bath patients will no longer be able to access the service.

We have a dedicated prescription team within the surgery who will continue to process your prescription requests via the routes listed above.

If you feel you are unable to order your own prescriptions or are the carer for somebody who is unable to order their own prescriptions, please ask your nominated pharmacy who can support you in ordering prescriptions.

Accessing Medication Out of Hours

If you require medication urgently, outside of our normal opening hours, there are several ways to request an emergency supply, even if you are away from home.

Medication Reviews

At least once a year, your repeat prescriptions will need to be reviewed by a GP, Pharmacist or Nurse Practitioner. You may be invited in for a consultation or this may be done remotely.

You may also be asked to attend the surgery for annual physical monitoring. This is usually aligned with your month of birth, however, there are times where you may be invited for interim monitoring, depending on your medical history and medication.

Your annual reviews are important to ensure the ongoing safety and efficacy of your medication. It is also an opportunity for you to discuss any concerns you may have and how to get the best outcome from taking the medication.

Please ensure you respond to any invitations as a failure to engage with your annual reviews may lead to a reduction in quantity of your medication, or in some instances, medication being withheld.

If you would like to request a review of your medication for any reason, please contact the surgery for this to be arranged.

Lost prescriptions

Once a prescription has been issued, the patient is responsible for its safekeeping.
Heart of Bath reserves the right to declines requests to replace lost prescriptions.
