NHS out of area patient scheme
Heart of Bath Medical Partnership offers the NHS out of area patient scheme to accommodate patients who reside outside our practice catchment area but wish to register/ remain registered with our practice.
As an out of area (OOA) patient you will have access to our clinicians during our normal surgery hours, 8am to 6pm, Monday to Friday. However, you are not eligible for GP home visits. Upon registration with the OOA scheme you will be sent further information regarding what to do if you develop an urgent illness or injury that prevents you from attending our surgery.
If in the future you wish to register with a GP practice in your local area, please register with your chosen surgery (this will automatically start your removal from our practice). Your local practice will then take over your care.
Current Heart of Bath patients
If you have been contacted regarding your out of area address and you wish to remain registered with HoB, you will need to be deducted and re-registered to be put on the out of area scheme (this process takes approximately 7 days to be completed by the local health authority). Please ensure you have all repeat medication to cover this transition period.
The NHS out of area scheme is a scheme we opt in to, this can be subject to change, which may result in you being asked to register locally in the future.
We accept/decline OOA registrations depending on the patient’s medical requirements and where their address is located (address must be within a reasonable distance from our practice). If you wish to discuss this further, please email us on bswicb.heartofbath@nhs.net
Registering with the surgery
Current processing time for registrations: 2 weeks.
For urgent registrations or nursing/care home registrations, do not submit an online form, please contact the practice directly on 01225 613 100 to discuss or email bswicb.heartofbath@nhs.net

Alternatively, you can visit the practice to complete paper registration forms.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
If this is your first time registering with the NHS you may not have an NHS number; please make sure to include your place of birth and first date of entry into the UK.
Please clearly complete every section/ box on the form. If you have consented to emails/SMS messages you will receive an email/SMS notification once you have been registered.
Moving home
We ask all our patients to keep their addresses and contact telephone numbers updated. You can do this by emailing us bswicb.heartofbath@nhs.net.
